Job summary

Rockford Center in Newark, DE is seeking a Registered Nurse (RN) for a Behavioral Health PRN position. The RN provides direct nursing care under physician supervision, delivering age-appropriate care in a 138-bed facility and coordinating with a multidisciplinary team.

An active Delaware nursing license (or eligibility) and CPR are required; two years of supervisory experience is preferred. A 2-week orientation is required, and staff benefit from a supportive leadership team and opportunities

Responsibilities

• Conduct ongoing and individualized nursing care duties as assigned and as dictated by the patient’s condition.
• When administering PRN medications, assess for confirmed med for PRN, documents need (rationale) and its effectiveness in the medical record.
• Reports change in patient physical, mental status or episodes of seclusion or restraint to nurse manager or supervisor during each shift.
• Ensures family and physician notification is completed and documented if a patient is transported to Emergency Room, Injured, Falls or is Placed in Seclusion or Restraints.
• Collaborates with other team members in the development, implementation, and evaluation of an individualized plan of care.
• Performs treatments and therapies as ordered by physician in an appropriate and timely fashion.
• Documents patient care, findings, and psychosocial responses to nursing interventions.
• Documents PRN medications given, reason and effect
• Initiates emergency measures according to age appropriate CPR and client resuscitation protocols.
• Participates in emergency crisis interventions according to age appropriate techniques.
• Maintains confidentiality related to patient, family, client facility and staff in accordance with HIPAA.

Position Summary

The Registered Nurse provides direct nursing care to patients as prescribed by the physician and is under the direction and supervision of the appropriate facility nursing leadership. The RN provides nursing care specific to the age population served. Nursing activities are limited to those in compliance with the State Nurse Practice Act and facility policies and procedures.

Requirements

Qualifications

• Associates or Bachelor's Degree with an emphasis in Nursing. Strong communication skills, excellent customer service and organization skills, highly effective listening skills, and strong clinical skills.
• Two (2) years of related Supervisory experience, with knowledge of psychiatric patient care techniques with understanding of mental illness, preferred.
• Current license to practice nursing in DE (or eligible to receive or renew).
• Current CPR
