Spring Creek Rehabilitation & Nursing Care Center in Brooklyn, NY, seeks an experienced RN Supervisor for per diem on-call coverage. You will lead nursing staff on weekday evenings (4:00 PM–12:00 AM) and weekends as needed, ensuring high-quality resident care and compliance with regulations.

Qualifications

• Active New York State RN license.
• Minimum of two years nursing experience.
• Prior supervisory or charge nurse experience preferred.
• Skilled nursing or long-term care experience preferred.
• Strong clinical judgement, leadership, communication, and organizational skills.

Responsibilities

• Oversee and coordinate resident care during assigned shifts.
• Supervise nursing staff and provide clinical guidance and support.
• Monitor resident conditions and respond promptly to changes in health status.
• Collaborate with physicians and interdisciplinary team members to support individualized care plans.
• Ensure compliance with facility policies, procedures, and applicable regulatory standards.
• Promote resident safety, quality care, and respectful communication with residents and families.
• Provide hands-on nursing care and clinical support as needed.
• Address nursing concerns and assist staff with problem-solving.
• Ensure accurate documentation and effective communication between shifts.
