Job summary

New Haven Center for Nursing and Rehabilitation, part of Essential Care, is seeking a dynamic RN supervisor for Per Diem shifts in New Haven, CT. You will lead the shift to ensure top-quality resident care and customer service, working closely with the DNS, ADNS, and nursing team.

The position focuses on strong communication with staff and management. The team recently celebrated compliance with the Annual Survey and is transitioning to a new facility with expansion plans in the works.

Qualifications

• RN license or eligibility to practice in CT.
• Ability to supervise and lead a nursing shift.

Responsibilities

• Oversee unit operations during the shift.
• Assist with treatment and medication administration.
• Assist with Admissions and coordinate patient intake.
• Attend and participate in weekly meetings.
• Serve as liaison between family, patient, doctor and other departments.
• Investigate incidents and follow-up as needed.
• Participate in managing staff and assignments to ensure quality of care.
• Contribute to patient condition discussions in weekly risk meetings.
• Assist with annual staff evaluations and participate in QAPI activities.

New Haven Center for Nursing and Rehabilitation, part of Essential Care, is located in a beautiful North End of New Haven minutes from I91 and I95, on the North Haven and North Branford town line. We are also in the process of building a state of art facility with 150 private rooms to replace our current location. The new building should be complete in approximately 2 years. Come join the team as we transition into the new facility and be part of something special.

We are looking for a dynamic RN supervisor to work with our newly hired DNS, ADNS, and experienced nursing team. This is a Per Diem Position. The right candidate will lead their shift to ensure top quality resident care and customer service. The right candidate must be a great communicator and communicate with staff and management alike.

We just completed our Annual Survey and had minimal deficiencies and no significant findings. We are also back in compliance.

RN Supervisor Responsibilities

• Oversee unit
• Assist with treatment and medication administration
• Assist with Admissions
• Attend and participate in weekly meetings
• Conduct daily audits of all new admission chartsServe as liaison between family, patient, doctor and other departments
• Investigate all incident reports and follow-up
• Participate in managing staff and assignments to ensure quality of care
• Participate and report on patient conditions in weekly risk meetings
• Participate in annual evaluations of staff
• Work as part of our QAPI team
