Job summary

Civita Care Center at West River in Milford, CT is seeking an experienced RN Supervisor to lead our nursing team and ensure high-quality resident care. This per diem role emphasizes clinical leadership, coordination of care, and adherence to professional standards within a collaborative interdisciplinary team.

You will supervise nursing staff, develop individualized plans of care, and work with physicians to adjust plans as needed.

Qualifications

• Possession of a valid unencumbered license as a Registered Nurse within the state of employment (CT).
• Current CPR certification.
• Basic computer skills (EHR familiarity).
• Preferred: experience with pain management and IV skills.
• Minimum 1 year of experience in Skilled Nursing with Charge Nurse, Unit Manager, or Supervisor experience.

Responsibilities

• Ensuring the delivery of effective, quality, outcome-focused nursing care
• Collaborating with residents, responsible parties, and interdisciplinary care teams to develop individualized plans of care
• Implementing the plan of care consistently and cost-efficiently, with a focus on resident-centered outcomes
• Utilizing all appropriate resources for resident/family/responsible party counseling and discharge planning
• Collaborating with physicians and other healthcare professionals to share relevant information for changing the plan of care
