Signature HealthCARE Bremen is seeking qualified Registered Nurses to join our team at a 97-bed facility providing short-term rehabilitation and long-term care. Qualifications

• Active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience (LTC preferred).
• Experience in a supervisory/charge nurse role preferred.
• Familiarity with electronic health records (EHR).

Responsibilities

• Provide direct nursing care to residents, including meds and wound care.
• Assist residents with ADLs while ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides on shift.
• Monitor residents’ status and notify physicians of changes.
• Develop and review resident care plans with the interdisciplinary team.
• Document accurately per facility policy and procedure.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities and staff education.
• Perform other duties as assigned by the Director of Nursing.
