Job summary

Signature HealthCARE of Bremen is seeking a dedicated Registered Nurse to provide direct patient care, assist with ADLs, and supervise nursing staff in a 97-bed setting. You will participate in care planning, documentation, and admissions while upholding safety and compassion.

The role requires an active RN license in Indiana, with preferred CPR/BLS and at least one year of clinical experience, especially in long-term care. Competitive benefits and a supportive team environment are available.

Qualifications

  • Current, active RN license in the state of practice.
  • CPR/BLS certification preferred (American Heart Association or American Red Cross).
  • Minimum one year of clinical nursing experience; long-term care experience preferred.
  • Experience in a supervisory or charge nurse role, preferred.
  • Familiarity with electronic health records (EHR) systems.
  • Proficiency with Microsoft Office applications.

Responsibilities

  • Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
  • Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
  • Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
  • Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
  • Develop, implement, and review individualized resident care plans with the interdisciplinary team.
  • Complete accurate and timely clinical documentation per facility policy and procedure.
  • Coordinate resident admissions, transfers, and discharges according to established procedures.
  • Participate in quality assurance activities, staff education, and regulatory surveys as needed.
  • Perform other duties as assigned by the Director of Nursing or designee.
