Job summary

Signature HealthCARE of Bremen is seeking a skilled RN to provide direct nursing care, administer medications, perform wound care, and support residents in a 97-bed facility offering short-term rehab and long-term care.

You will supervise nursing assistants, coordinate admissions and discharges, document care plans, and participate in quality assurance as part of a compassionate interdisciplinary team focused on resident-centered care and quality of life.

Qualifications

• Current, active RN license in the state of practice.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs and ensure comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during shifts.
• Monitor residents' status and notify physicians of changes.
• Develop and review individualized care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and staff education.
