Job summary

RiverSpring Licensed Home Care Services Agency seeks an experienced Registered Nurse (RN) Supervisor for a multi-setting clinical role on an immediate per diem basis in the Bronx, NY. The RN will coordinate nursing services across ALR, LHCSA/home care, and SNF, perform assessments, and supervise staff to ensure safe transitions and high-quality resident care.

Ideal candidates hold an NYS RN license (BSN preferred) with 1+ year in long-term care or related settings and CPR/BLS certification.

Qualifications

• Graduate of an accredited school of professional nursing with a current, unrestricted New York State license and registration as a Registered Professional Nurse.
• Bachelor of Science in Nursing (BSN) preferred.
• Minimum one year of nursing experience in long-term care, assisted living, home care, acute care, or post-acute care preferred.
• Prior supervisory or charge nurse experience preferred.
• CPR/BLS certification as required by organizational policy.
• Strong communication, organization, clinical judgment, computer, and documentation skills.
• Ability to travel between assigned settings and conduct home visits when required.

Responsibilities

• Provide and coordinate professional nursing services across the Assisted Living Residence, LHCSA/home care, and SNF based on resident/patient needs.
• Perform clinical assessments and reassessments, develop and evaluate care plans, and provide direct nursing care.
• Supervise and support nursing and paraprofessional staff and coordinate with providers and interdisciplinary teams.
• Promote safe transitions across the continuum and ensure clinical quality and regulatory compliance.
