Job summary

RiverSpring is seeking a Licensed Practical Nurse (LPN) Supervisor for a dynamic multi-setting clinical role in Bronx, NY. The role oversees nursing staff across Assisted Living and Skilled Nursing settings, ensuring adherence to clinical standards and high-quality resident-centered care.

Responsibilities include supervising CNAs/HHA staff, developing care plans, documenting thoroughly, administering medications, and coordinating with providers.

Qualifications

• Licensed Practical Nurse with NYS license.
• Minimum 6 months in long-term care or assisted living preferred.
• Supervisory experience is a plus but not required.
• Strong communication and documentation skills are required.

Responsibilities

• Supervise CNAs and Home Health Aides, coaching and guiding to ensure quality care and workflow across settings.
• Assist in developing, implementing, and evaluating individualized nursing care plans.
• Observe, record, and report symptoms or changes to the clinical leader or provider.
• Administer medications and treatments within licensure scope and document accurately.
• Participate in assessments and reassessments to determine clinical status and service needs.
• Maintain accurate clinical records and prepare reports to support quality and regulatory compliance.
• Collaborate with interdisciplinary team to support continuity of care and service excellence.
• Communicate with providers to secure orders and address medication concerns.
• Maintain current clinical competency and provide training to paraprofessional staff.
