Job summary

Lone Tree Post Acute in Colorado is seeking a dedicated and compassionate RN to join our team on a full-time basis. The RN will supervise day-to-day nursing activities, ensure compliance with regulations, and collaborate with the DON and ADON to deliver high-quality resident care.

Responsibilities include admissions/transfers/discharges, charting and documentation, care planning, and maintaining accurate records, while promoting continuity of care across departments within a supportive

Qualifications

• Current and valid Registered Nurse (RN) license.
• Experience in long-term care or skilled nursing preferred.
• Strong clinical, organizational, communication and leadership skills.
• Ability to work effectively as part of a multidisciplinary team.
• Commitment to compassionate, high-quality resident care.

Responsibilities

• Supervise and coordinate nursing activities during the assigned shift.
• Ensure nursing staff follow facility policies, procedures and regulations.
• Assist with resident admissions, transfers and discharges.
• Monitor and support the delivery of quality resident care.
• Complete required nursing documentation, charting, reports and forms.
• Complete accident and incident reports as necessary.
• Participate in resident care planning and discharge planning.
• Collaborate with nursing staff and other departments to promote continuity of care.
• Attend and participate in nursing staff meetings and planning activities.
• Maintain accurate and timely records and documentation.
