Responsibilities
- Conduct comprehensive patient assessments to determine health status and needs.
- Develop, implement, and evaluate individualized care plans based on patient assessments.
- Administer medications and treatments as prescribed by physicians while monitoring for effectiveness and side effects.
- Provide education and support to patients and their families regarding health management and disease prevention.
- Collaborate with interdisciplinary teams to coordinate patient care across various settings, including medical offices and acute care facilities.
- Maintain accurate medical records and documentation in accordance with regulatory standards.
- Participate in health coaching initiatives to promote wellness within the community.

Skills
- Strong knowledge of hospital protocols and procedures, particularly in medical-surgical environments.
- Experience in home community care settings is highly desirable.
- Proficient in acute pain management techniques and interventions.
- Familiarity with medical imaging processes is a plus.
- Excellent communication skills for effective interaction with patients, families, and healthcare teams.
