Delta Community Supports, Inc. in Warminster, PA is seeking a Registered Nurse to join our team. You will supervise LPNs, provide medical oversight in community settings, and coordinate with physicians to deliver person-centered care.

The role requires PA RN licensure, an ASN or BSN, strong communication, and a commitment to independent living and inclusion for individuals with developmental disabilities.

Qualifications

• ASN is required for this role.
• Current Pennsylvania RN license is required.
• Valid driver’s license.
• Strong communication, organizational, and clinical skills.

Responsibilities

• Supervise and support a small team of LPNs.
• Provide medical oversight and support for individuals in community-based settings.
• Act as liaison with physicians and other healthcare providers.
• Review and manage MARs, medication orders, and supplies.
• Conduct staff training on medical conditions, procedures, and equipment.
• Develop medical and feeding protocols with healthcare professionals.
• Monitor health concerns and ensure timely follow-up.
• Coordinate psychiatric care and support care plan implementation.
• Maintain accurate and up-to-date documentation.
• Serve as a medical resource for staff and ensure best practices in care.
• Assist state licensing and compliance activities.
