Job summary

Rocky Mountain Care seeks a Registered Nurse to lead nursing care in our Assisted Living facility in Utah. You will supervise CNAs, ensure regulatory compliance, and coordinate resident care with the interdisciplinary team.

The RN must hold an active nursing degree, an unrestricted RN license, and CPR certification, with duties including medication administration, care planning, and end-of-life care involvement. On-site position emphasizing teamwork and resident dignity.

Qualifications

• RN license in practicing state is required.
• Nursing degree from an accredited college or university is required.
• Current CPR certification is required.

Responsibilities

• Directs daily activities of CNAs and caregivers per regulations and facility policies.
• Ensures compliance with nursing policies and procedures.
• Participates in admission, discharge and transfers of residents.
• Assess changes in resident status and notify physician and family as needed.
• Prepare and administer medications as per physician orders and observe for adverse effects.
• Schedule follow-up appointments and transportation as required.
• Provide nursing leadership to staff in the Assisted Living.
• Participate in QAPI activities and infection control initiatives.
• Maintain Change of Condition Log and proper medication destruction procedures.
• Ensure timely transitions to home health, hospice, or palliative services as appropriate.
