Job summary

Swan Lake Nursing & Rehabilitation in Patchogue, New York is seeking an RN Supervisor to join our leadership team. In this role, you will supervise LPNs and CNAs, oversee resident care documentation, and help develop care plans to support our residents' well-being.

You will establish a compassionate environment, ensure compliance with state and federal regulations, and address questions from residents and families. The position offers a consistent evening shift and full-time hours.

Qualifications

• Long-term Care experience preferred.
• Valid RN license required.
• 2–4 years of management experience.
• Knowledge of City, State, and Federal regulations pertaining to licensed skilled nursing facilities.

Responsibilities

• Establish a compassionate environment by supporting Residents and families.
• Maintain documentation of Resident care services by auditing Resident and department records.
• Create appropriate care plans for residents.
• Handle all supervisory duties for assigned units, including overseeing LPNs and CNAs that report to you.
• Help establish and implement employment policies and procedures.
• Maintain the standards of care for the units.
• Administer medication as needed, review Resident records, and monitor the quality of care.
• Address questions or complaints brought forward by Residents or their families.
