Responsibilities

• Supervise a team of registered nurses, licensed practical nurses, and certified nursing assistants to ensure residents receive high-quality care.
• Develop and implement individualized care plans for each resident.
• Collaborate with the interdisciplinary team to address medical, emotional, and spiritual needs of residents.
• Conduct regular rounds to assess care quality and identify improvement opportunities.
• Provide guidance and support to nursing staff to ensure necessary resources and tools are available.
• Maintain positive relationships with residents, families, and healthcare providers to keep all parties informed and satisfied with care.
• Participate in Quality Improvement initiatives to improve patient outcomes and reduce readmissions.
• Maintain accurate records of patient care and communicate effectively with the interdisciplinary team.
