Job summary

Capital Health in New Jersey is seeking an experienced RN to oversee the implementation and integration of national standards and evidence‑based clinical practice guidelines for stroke patients.

You will coordinate care across the continuum, mentor staff, and ensure readiness for regulatory surveys while promoting best‑practice outcomes within a 40‑hour work week.

Qualifications

  • Master's in Nursing (MSN) or Bachelor's in Nursing (BSN) required; MSN must be attained within 18 months of hire.
  • Minimum 2 years of stroke unit experience or stroke patient care.
  • SCRN, ASC-BC preferred.
  • AHA BLS – Healthcare Provider; NJ RN license.
  • NIH Stroke Scale certification (or must obtain by orientation end) and 8 hours of annual stroke-related CNE.
  • ACLS/NRP/PALS certifications required within specified timeframes depending on role.

Responsibilities

  • Coordinates disease-specific care from arrival to discharge across the continuum.
  • Implements care protocols and revises supplies/equipment accordingly.
  • Monitors hospitalized patients and documents outcomes; educates patients and families.
  • Mentors staff to implement high-quality stroke care standards.
  • Prepares reports and supports regulatory surveys and inspections.
  • Leads performance improvement efforts and facilitates staff education.

Position Overview

Opens a full‑time equivalent (1.0 FTE) position. Compensation may differ depending on assigned hours and position status (e.g., part‑time).

$86,528.00 – $113,630.40

Job Summary

Oversees implementation and integration of national standards and evidence‑based clinical practice guidelines in the care of stroke patients.

Minimum Requirements

Education: Master’s in Nursing (MSN) or Bachelor’s in Nursing (BSN) and must attain MSN within 18 months of hire.

Experience: Minimum of 2 years of experience on a Stroke Designated unit or working with stroke patients. SCRN, ASC‑BC preferred.

Other Credentials: AHA BLS – Healthcare Provider, Registered Nurse – NJ.

Required Certifications: NIH Stroke Scale Certification (or must obtain by end of orientation) AND 8 hours of stroke‑related continuing nursing education (CNE) contact hours annually.

CPR Requirements:

  • Requires ACLS (or must obtain within 6 months of hire date) if assigned to: Critical Care/Intermediate/Telemetry, Emergency Rooms, Pediatrics/Pediatrics Emergency Room, Labor & Delivery, Surgical Services (not to include Perioperative), Interventional Procedures, Observation.
  • Requires NRP (or must obtain within 6 months of hire date) if assigned to: Maternity Services.
  • Requires PALS (or must obtain within 6 months of hire date) if assigned to: Emergency Rooms, Infant Follow‑Up, Surgical Services (only PACU & Same Day Surgery), Pediatrics/Pediatric ED.
Knowledge and Skills

Possesses excellent organizational, interpersonal, conflict resolution, and verbal and written communication skills. Ability to effectively manage multiple projects simultaneously and respond quickly in a fast‑paced environment.

Special Training

Intermediate computer skills in Microsoft Office and extensive familiarity with use of the internet as a clinical and administrative resource.

Ability to form and maintain interpersonal professional working relationships.

Essential Functions
  • Manages and coordinates disease‑specific care from arrival to discharge and across the entire continuum, responding to alerts and collaborating with the healthcare team to promote timely access to care.
  • Manages implementation of care protocols as defined by Capital Health, coordinating revisions and changes to all related supplies and equipment.
  • Monitors patients while hospitalized until successful discharge, including recording daily disease‑specific census data and facilitating education for patients and families.
  • Assesses, manages, implements, and evaluates interventions required by state, national regulatory bodies, CMS and the Joint Commission; mentors and educates staff to promote successful implementation of appropriate care standards.
  • Maintains readiness for system‑wide regulatory surveys and inspections, collecting data and reviewing records to meet quality and outcome criteria.
  • Engages staff in performance improvement activities, developing action plans to achieve best‑practice goals and sustainability processes for patient care.
  • Maintains outcomes and quality improvement activities by rounding on patients to track individual outcomes with the Nurse Manager, Medical Director, Quality Management Representative, and team members; supports clinical research when appropriate.
  • Reviews complaints and resolves problems with discretion and judgment; provides counseling and education to patients and significant others.
  • Serves as liaison between the Emergency Department and inpatient units to facilitate transfers and admissions, coordinating with therapists, Care Management, and rehabilitation/nursing home evaluators to ensure timely transitions; collaborates with physicians to ensure quality care.
  • Prepares accurate reports within established guidelines and timelines, ensuring professional communication.
  • Delegates, supervises, and provides clinical support to staff, prioritizing interventions and establishing treatment goals; conducts education needs assessments and develops competency assessments.
  • Conducts in‑service meetings, seminars, and conferences for physicians, staff, and patients on prevention and health management; facilitates disease‑specific educational programs and participates in onboarding of medical staff.
