Job summary

Qualifications

Responsibilities

• Conduct phone screenings to assess clinical eligibility for services.
• Document interactions in the electronic medical record per guidelines.
• Provide plan enrollment information to clients and families.
• Offer state and local health resources for enrollment and support.
• Attend meetings related to the role and responsibilities.
• Meet continuing education requirements mandated by the Florida Board of Nursing.
