Job summary

Sweetwater Care in Simi Valley, CA is hiring a Registered Nurse (RN) to join our team serving individuals with disabilities in small Intermediate Care Facilities (ICF). You will provide skilled nursing care, implement and oversee health care plans, administer medications, and collaborate with a multi-disciplinary team to support residents’ well-being, with on-call evenings, weekends and holidays as needed.

Required California RN license and CPR certification; excellent communication, teamwork,

Qualifications

• CA RN license is required.
• Minimum of 1 year RN experience.
• CPR certification required.

Responsibilities

• Provide skilled nursing care to individuals with disabilities in an ICF setting.
• Implement and oversee health care plans for residents.
• Administer medications and perform medical procedures as needed.
• Collaborate with a multi-disciplinary team to ensure residents' well-being.
• Maintain accurate medical records and documentation.
• Support and guide LVN and DSP staff.
• Participate in complex case management and pursue ongoing learning.
