Responsibilities

• Provide skilled nursing care to individuals with disabilities in an ICF setting
• Implement and oversee the health care plans for residents
• Administer medications and perform medical procedures as needed
• Collaborate with a multi-disciplinary team to ensure resident well-being
• Maintain accurate medical records and documentation
• Offer support and guidance to LVN and DSP
• Participate in complex case management and ongoing learning opportunities
