Job summary

Sapphire Health Services, LLC in Myrtle Point, OR is seeking a dedicated Registered Nurse (RN) to join our Skilled Nursing Facility team. The RN will coordinate with an interdisciplinary group, assess residents, implement care plans, and supervise nursing staff to ensure high-quality, person-centered care.

Responsibilities include administering medications, documenting in EHR, leading care plan meetings, and ensuring regulatory compliance.

Qualifications

• RN licensure in the state of practice.
• Experience in skilled nursing or long-term care preferred.
• Strong communication and organizational skills.
• Ability to work effectively in a multidisciplinary team.

Responsibilities

• Conduct comprehensive resident assessments at admission and on change of condition.
• Participate in care plan meetings with residents and families.
• Provide direct nursing care including medications, treatments, wound care, and mobility assistance.
• Monitor vital signs, document health status, and respond to changes.
• Supervise LPNs, CNAs, CMAs, and other staff as assigned.
• Maintain accurate MARs and ensure proper medication handling.
• Ensure compliance with care plans, standards, and regulatory requirements.
