Sapphire Health Services in Sheridan, Oregon, is seeking a dedicated Registered Nurse (RN) for our Skilled Nursing Facility to deliver high-quality, person-centered care. You will coordinate with an interdisciplinary team, assess resident needs, and implement individualized care plans while ensuring regulatory compliance and safe medication administration.

The role requires RN licensure in Oregon, strong geriatric care knowledge, excellent communication, and the ability to work with nursing

Qualifications

• RN licensure in the State of practice (Oregon).
• Experience in skilled nursing, long-term care, or acute care preferred.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work effectively on an interdisciplinary team.
• Proficiency with electronic health records (EHR) and basic computer skills.
• Ability to perform physically demanding tasks and assist residents with mobility.

Responsibilities

• Conduct comprehensive resident assessments at admission and as conditions change.
• Participate in care plan meetings with residents, families, and care team.
• Provide direct nursing care including medications, treatments, wound care and interventions.
• Monitor and document health status, vital signs, and responses to treatment.
• Assist with activities of daily living as needed.
• Adhere to care plans and nursing standards of practice.
