Job summary

Stellar Care Group is seeking an experienced STAFF NURSE for our Skilled Nursing Facility. The role requires Ohio RN licensure and focus on safe medication administration, resident assessments, and coordinated care with physicians and families.

The nurse will document care, participate in patient care planning, and support admissions, discharges, and transfers while maintaining empathetic resident relations. This is a full-time, on-site position with standard nursing shifts.

Qualifications

• Must be an RN currently licensed in the State of Ohio.
• Experience with elderly and disabled populations and skilled nursing care desired.
• Proficient in documentation, record keeping and adherence to care plans.

Responsibilities

• Abide by the Ohio nursing board scope of practice.
• Administer medications and treatments as ordered by physician.
• Document accurately on administration records and nursing policies.
• Assist with admissions, discharges and transfers of residents.
• Communicate changes to physicians, care providers and families as appropriate.
• Review orders for accuracy of medication administration and supply needs.
• Maintain medication carts/rooms and ensure correct storage and temperature.
• Provide leadership to nonprofessional staff on unit and educate residents in self-care.
