Job summary

Signature HealthCARE of Monteagle Rehab & Wellness Center is seeking a licensed LPN/LVN to provide direct nursing care, medication administration, and daily support for residents. You will supervise nursing staff, coordinate care plans, and document thoroughly within the facility's EHR system.

Strong communication and regulatory knowledge are essential for delivering high-quality care. Minimum of one year of clinical nursing experience, with LTC preferred; CPR/BLS certification is preferred.

Qualifications

Active LPN/LVN license in state of practice.
CPR/BLS certification preferred.
Minimum 1 year clinical nursing experience; long-term care preferred.
Supervisory or charge nurse experience preferred.
Strong clinical observation, medication administration, and documentation skills.
Knowledge of Federal and State long-term care regulations.
Proficiency with EHR systems and MS Office.

Responsibilities

Provide direct nursing care to residents, including medication administration and wound care.
Assist residents with ADLs and ensure comfort, dignity, and safety.
Supervise and direct nursing assistants and medication aides during shifts.
Monitor residents’ status and notify physician of changes.
Develop and review individualized care plans with the team.
Complete accurate clinical documentation per policy.
Coordinate admissions, transfers, and discharges per procedures.
Participate in quality assurance, staff education, and regulatory surveys.
