Qualifications

• Active RN license.
• CPR/BLS Certification required.
• Experience in long-term care or skilled nursing preferred.
• Excellent technical, assessment and documentation skills.
• Strong analytical and problem-solving skills.
• Ability to lift 25+ pounds.
• Stand, bend and walk for extended periods.
• Team player with strong communication and compassionate senior care mindset.

Responsibilities

• Provide nursing care to meet resident needs per standards and physician orders.
• Plan and monitor nursing interventions based on individual resident needs.
• Evaluate care effectiveness and adjust interventions as needed.
• Conduct family and resident education.
• Administer medications and treatments per center policy.
• Transcribe physician orders and chart progress accurately.
• Identify and report changes in condition to supervisor, physician and family.
• Coordinate rehabilitation programs with therapists to support outcomes.
