Job summary

Qualifications

• Colorado RN license and CPR certification required.
• 1–3 years' direct care experience preferred.
• Ability to follow care plans and physician orders.
• Strong teamwork and communication with interdisciplinary staff.

Responsibilities

• Plans and delivers nursing care per care plan and doctor's orders.
• Monitors residents' condition and responds to changes.
• Coordinates admissions, transfers and discharges with required documentation.
• Ensures nursing history is complete in medical records.
• Collaborates with the facility interdisciplinary team.
