Job summary

Ebenezer is seeking a Registered Nurse (RN) to provide personal care and health management for residents at Meadowwoods Senior Living in Bloomington, MN. The role includes assessments, care planning, medication oversight, and collaboration with care teams.

The position involves a two-week, 72-hour schedule with evening shifts and every third weekend, in a resident-centered environment managed by Ebenezer. Nursing licensure and BLS are required.

Qualifications

• Associates Degree in Nursing is required.
• MN RN license is required.
• Basic Life Support (BLS) certification required.
• Less than 1 year nursing experience.

Responsibilities

• Maintains current assessment and documentation of resident health status.
• Performs initial and ongoing assessments per Minnesota Assisted Living regulations and Nurse Practice Act.
• Collaborates with residents and families to ensure appropriate services and placement.
• Develops and maintains emergency plans including POLST.
• Maintains clinical charts and progress notes using computerized documentation.
• Coordinates resident health services with internal and external providers.
• Ensures physician orders are signed and implemented per regulations.
• Oversees medication administration and ensures timely ordering and renewal of meds.
