Job summary

PruittHealth in Georgia seeks a Registered Nurse to coordinate admissions for hospice/SOURCE care, and to outreach referral sources. You will review patient needs, manage paperwork, and collaborate with physicians, therapists, and social services to ensure smooth transitions.

The role requires 5+ years nursing experience, CPR certification, a valid driver’s license, and willingness to travel within the region.

Qualifications

Must hold a State-licensed Registered Nurse (RN)
Valid driver’s license and automobile insurance liability
CPR certification
Minimum of five (5) years of nursing experience; Hospice or SOURCE care preferred

Responsibilities

Markets their area for SOURCE and PRN meetings with new PCPs.
Completes all new admission paperwork for signed forms and in-home assessments.
Participates in weekly team meetings with Medical Director and presents admit information and discharges.
Completes quarterly home visits and PCP visits with all requirements.
Tracks hospital/ER visits and coordinates with hospitals and records.
Uses own vehicle for travel and arranges transportation as needed.
Attends trainings, may train new hires, and participates in on-call rotations.
