Job summary

PruittHealth Hospice is hiring a Flex RN Case Manager to support admissions and ongoing palliative care visits in Georgia. The role involves coordinating care with the physician and interdisciplinary team, scheduling visits, and ensuring timely documentation.

Hours are 9a-6p or 10a-7p, with home or facility-based care. Qualified candidates hold an active RN or LPN license in Georgia, with 3+ years of nursing experience; palliative/hospice or home health experience is preferred.

Qualifications

• Active RN or LPN licensure in Georgia.

Responsibilities

• Take and process palliative care referrals.
• Provide support to Palliative Care Nurse Practitioner and Medical Director.
• Schedule visits to support goals of care as ordered by MD.
• Coordinate and communicate patient care goals between providers.
• Make calls to present the palliative program and assess interdisciplinary needs.
• Ensure communication of assessment findings to attending physician.
• Complete documentation accurately and timely.
• Inform palliative team of new admissions and discharges.
• Coordinate referrals using the referral process.
• Serve as a liaison for medical directors and nurse practitioners.
• Run billing reports and address validation holds/billing errors.
