About the Role:

We are seeking a compassionate and experienced Registered Nurse to join our nursing team. As an RN with Tucker House, you will be responsible for providing high-quality patient care, working collaboratively with our interdisciplinary team, and contributing to a positive and supportive work environment. In this role, you will have the opportunity to make a meaningful difference in the lives of our residents and their families.

Responsibilities:

• Provide direct patient care, including administering medications, performing treatments, and educating patients and families on disease management and self-care
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Monitor and report patient progress, identifying areas for improvement and implementing strategies to optimize outcomes
• Participate in quality improvement initiatives, incorporating best practices and evidence-based guidelines into practice
• Collaborate with other healthcare professionals to ensure seamless transitions and comprehensive care
• Maintain accurate and timely documentation of patient care, including progress notes and med administration records
