Job summary

Northeast Healthcare Recruitment, Inc. (NEHCR) seeks a Registered Nurse (RN) to deliver patient-centered care through in-home visits for Medicare patients in Pennsylvania. The role focuses on urgent and episodic interventions and coordinates with physicians, NPs and care teams across the continuum.

Responsibilities include initial assessments, follow-up visits, medication reviews, wound care, patient education, and documentation. Schedule is 10am-7pm EST; mileage and full benefits are provided.

Qualifications

  • Active State Licensure required; compact licensure preferred.
  • 3+ years as an RN in a direct, patient-facing care delivery role (required).
  • CPR Certification
  • Prior hospice, palliative care, or geriatrics experience preferred.

Responsibilities

  • Deliver urgent, in-home patient care under the direction of a licensed provider throughout the assigned area including initial assessments, routine follow-up visits, and ongoing care coordination.
  • Conduct comprehensive home safety evaluations and document findings during patient visits.
  • Deliver patient care under the direction of a licensed provider by obtaining patient histories.
  • Review medications and treatment plans.
  • Perform physical assessments.
  • Identify and address health concerns observed or reported during home visits.
  • Perform clinical interventions as appropriate and at the direction of the overseeing provider, including medication administration (PO, SL, IM, IV), wound care, point-of-care testing, phlebotomy, Foley catheter care, IV fluid administration, and other approved nursing procedures.
  • Maintain accurate, timely, and complete documentation.
  • Provide patient and caregiver education.
  • Support care transitions, end-of-life discussions, palliative care interventions, and closures of preventive care and HEDIS quality gaps, including screenings and medication adherence initiatives.
  • Participate in Interdisciplinary Team (IDT) meetings, providing updates and insights from patient and family interactions to support care planning.
  • Scheduled working hours: 10am-7pm EST (tentative).

Salary: $ 77,000 – $ 112,00 depending onexperience plus Mileage and Full Benefits package

Requirements
