Job summary

Northeast Healthcare Recruitment, Inc. (NEHCR) seeks a Registered Nurse (RN) to deliver patient-centered in-home care for Medicare patients in the assigned area. You will work with physicians, NPs and care teams to address needs across the care continuum, including urgent interventions and care coordination.

Requirements include an active RN license, CPR certification, and 3+ years of direct patient-facing RN experience. Hospice/palliative care or geriatrics experience is strongly preferred.

Responsibilities

  • Deliver urgent in-home patient care under the direction of a licensed provider, across the assigned area.
  • Conduct comprehensive home safety evaluations and document findings during visits.
  • Obtain patient histories, review medications and treatment plans, and perform physical assessments.
  • Identify and address health concerns observed during home visits; perform nursing interventions as directed.
  • Administer medications (PO, SL, IM, IV), provide wound care, POCT, phlebotomy, Foley care and IV fluids as appropriate.
  • Maintain accurate, timely documentation and provide patient and caregiver education.
  • Support care transitions, palliative care interventions, and closing of preventive care gaps.
  • Participate in Interdisciplinary Team (IDT) meetings to support care planning.
  • Schedule: 10am-7pm EST (tentative).

Job description

Weareseeking Registered Nurse RN to deliver patient centered care throughin home visits for Medicare patients.

Thisrole primarily supports urgent and episodic interventions, working inpartnership with Physicians, NPs and interdisciplinary care teams to address arange of patient needs across the care continuum.

Salary : $ 77,000 – $ 112,00 depending onexperience plus Mileage and Full Benefits package

  • Deliver urgent,in-home, patient care under the direction of a licensed providerthroughout the assigned area including initial assessments, routinefollow-up visits, and ongoing care coordination.
  • Conduct comprehensivehome safety evaluations and document findings during patient visits.
  • Deliver patient careunder the direction of a licensed provider by:
  • Obtaining patienthistories
  • Reviewingmedications and treatment plans
  • Performingphysical assessments
  • Identifying andaddressing health concerns observed or reported during home visits
  • Perform clinicalinterventions as appropriate and at the direction of the overseeingprovider, including medication administration (PO, SL, IM, IV), woundcare, point-of-care testing, phlebotomy, Foley catheter care, IV fluidadministration, and other approved nursing procedures.
  • Maintain accurate,timely, and complete documentation
  • Provide patient andcaregiver education
  • Support caretransitions, end-of-life discussions, palliative care interventions, andthe closure of preventive care and HEDIS quality gaps, includingscreenings and medication adherence initiatives.
  • Participate inInterdisciplinary Team (IDT) meetings, providing updates and insights frompatient and family interactions to support care planning
  • Scheduled workinghours: 10am-7pm EST (tentative)
Requirements:
  • Active StateLicensure required; compact licensure preferred.
  • 3+ years as an RNin a direct, patient-facing care delivery role (required).
  • CPR Certification
  • Previousexperience in hospice, palliative care, or geriatrics strongly preferred; othercomplex patient health care experience will be considered.
