Job summary

Generations Hospice is seeking an On-Call RN to provide 1:1 hospice care with lower patient-to-nurse ratios and strong interdisciplinary team support. You will work after-hours to deliver compassionate care, assess symptoms, administer medications, and communicate with physicians to adjust care plans.

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or similar setting preferred.

Responsibilities

• Collaborate with Remote Triage On Call Team to deliver hands-on hospice care during after-hours.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medication or adjust care per hospice protocols or orders.
• Communicate with physician regarding changes in condition and obtain orders as necessary.
• Provide guidance to patient and family about current condition and next steps.
• When death occurs during on-call hours, pronounce death and notify hospice team.

Schedule: Full Time 7ON 7OFF Weekdays Monday-Thursday 430p-8a & Friday starting at 4:30PM through Monday 8AM

As a On-Call RN You will:

• Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis
• Assess and manage urgent symptoms quickly and compassionately
• Administer medication or adjust care based on hospice protocols or provider orders
• Communicate with the physician regarding changes in condition and obtain orders as necessary
• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps
• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team
• Complete timely documentation and communication

Qualifications
