At Home Care Hospice, an affiliate of Adoration Health, is seeking an On-Call Registered Nurse to join our team in Fredericksburg, VA. The role involves 7 on, 7 off night rotation with 1:1 patient care and after-hours support for families within the Fredericksburg and surrounding areas.

Requirements include state-licensed RN, hospice experience preferred, and a valid driver's license. Competitive benefits and stable opportunities offered.

Qualifications

• Graduate of an accredited nursing school with current state licensure.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with insured vehicle.

Responsibilities

• Collaborate with the on-call team to deliver hands-on hospice care during after-hours.
• Assess and manage urgent symptoms quickly and compassionately.
• Administer medications or adjust care per hospice protocols.
• Communicate changes in condition with the physician and obtain orders as needed.
• Support patients and families in understanding current condition and next steps.
• Pronounce death when applicable during on-call hours and provide family support.
• Complete timely documentation and communication.

As a On-Call RN You will:

• (CHOOSE ONE: If office HAS a Remote Triage team) Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis

• (CHOOSE ONE: If office DOES NOT have Remote Triage) Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis

• Assess and manage urgent symptoms quickly and compassionately

• Administer medication or adjust care based on hospice protocols or provider orders

• Communicate with the physician regarding changes in condition and obtain orders as necessary

• Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps

• If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team

• Complete timely documentation and communication
