Job summary

Hospice of North Alabama in Florence, AL is seeking a Hospice Admissions RN to join our team. You will visit patients referred for hospice care at home or facilities, explain services, obtain consents, and coordinate the start of services with the interdisciplinary team.

Graduate nursing licensure in the operating state is required; one year hospice experience is required, CPR certification is needed, and you must have a valid driver's license with auto insurance.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year hospice nursing experience required, 3+ years preferred or similar setting will be considered in place of hospice nursing experience.
• Must be a licensed driver with an automobile that is insured in accordance with state requirements and in good working order.
• Possess and maintain current CPR certification in states where required.

Responsibilities

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents
• Obtain medical history, current medications, and symptoms
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications.)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
