Job summary

Hospice of North Alabama is seeking a Hospice Admissions RN to join our Florence, AL team, serving Franklin, Winston, Marion, Fayette, and Lamar counties. This full-time role offers a Monday–Friday, 10am–7pm schedule and patient visits in homes and facilities.

You will assess patients, explain hospice services, obtain consent, and coordinate start of services with the care team, while maintaining meticulous documentation and ensuring quality care.

Qualifications

• Must be licensed to practice as an RN in Alabama.
• Minimum of 1 year hospice nursing experience; 3+ years considered.
• Valid driver’s license and insured vehicle.
• CPR certification maintained.

Responsibilities

• Visit patients at home, hospitals, or nursing facilities for hospice admissions.
• Explain services, discuss goals, obtain consents and medical history.
• Perform comprehensive assessments and determine eligibility for hospice.
• Coordinate start of services and collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.

As a Hospice Admissions RN You will:

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents
• Obtain medical history, current medications, and symptoms
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications.)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
