Job summary

Adoration Health in York, PA is seeking a Hospice Admissions RN to provide patient-centered admissions coordination, visiting patients in homes, hospitals, and facilities, and guiding families through hospice options.

You will obtain medical histories, perform assessments to determine hospice eligibility, coordinate the start of services, and collaborate with the interdisciplinary team while maintaining timely documentation.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as LTC/ICU/ED will be considered in place of hospice.
• Must be a licensed driver with insured vehicle in good working order.

Responsibilities

• Visit patients (homes, hospitals, nursing facilities, etc.) referred for hospice care.
• Explain hospice services, discuss goals and care with patients and families, answer questions, obtain consents and documents.
• Obtain medical history, current medications, and symptoms.
• Perform comprehensive clinical assessments to determine eligibility for hospice per Medicare/insurance guidelines.
• Coordinate the start of services (nursing, aides, equipment, medications).
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.

As a Hospice Admissions RN You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
