Job summary

Adoration Health, a BrightSpring Health Services affiliate, is seeking a Home Health Registered Nurse to join our Baltimore team. This PRN role offers flexible scheduling and 1:1 patient care in patients' homes, with support from an interdisciplinary team.

You will assess needs, develop care plans, administer medications, educate families, and maintain thorough documentation while driving quality outcomes in the community setting.

Qualifications

• RN license in the state of practice and residence is required.
• 1 year nursing practice in a patient care setting; home health/geriatrics preferred.
• Current CPR certification.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligning with orders and patient goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient families and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by the local branch.

Qualifications

• Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence is required
• One year nursing practice in a patient care setting required; and home health, geriatrics or other related settings preferred
• Valid driver's license, acceptable driving record, and proof of car insurance in accordance with Adoration policy
• New nursing graduates may be considered in select markets based on program availability
• Current CPR certification
