Job summary

At-Heart Home Care & Hospice in Virginia Beach, VA is seeking a dedicated Home Health Registered Nurse to join our team. This role offers 1:1 patient care, flexible scheduling options, and opportunities for professional growth within a supportive environment.

Responsibilities include creating care plans, administering medications, providing symptom management, and collaborating with an interdisciplinary team.

Qualifications

• Registered Nursing Degree (Associate or Bachelor) with current unrestricted license in the state of practice.
• One year nursing practice in a patient care setting.
• Valid driver's license and proof of car insurance as per policy.
• New nursing graduates may be considered in select markets based on program availability.
• Current CPR certification.

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligned with MD orders and patient goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient's family and caregivers.
• Collaborate with an interdisciplinary team and maintain timely documentation.
• Participate in on-call rotation as required by the local branch.

Responsibilities

As a Home Health Registered Nurse, You will:

Qualifications

• Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence is required


• One year nursing practice in a patient care setting required; and home health, geriatrics or other related settings preferred


• Valid driver's license, acceptable driving record, and proof of car insurance in accordance with Adoration policy


• New nursing graduates may be considered in select markets based on program availability


• Current CPR certification
