Job summary

At-Heart Home Care & Hospice in Virginia Beach, VA is seeking a compassionate Hospice Registered Nurse to join our team. Provide 1:1 patient care, develop and implement care plans, and support families through difficult times in a warm, patient-centered environment.

We value clinical excellence, thorough documentation, and teamwork. PRN M-F 8-5 with opportunities for advancement and stable employment within a growing organization.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with a valid automobile and insurance.
• Possess CPR certification in states where required.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patient’s family and caregivers.
• Collaborate with interdisciplinary team; maintain accurate documentation.
• Participate in on-call rotation as required by local branch.
