Job summary

At-Heart Home Care & Hospice in Virginia Beach, VA is seeking a dedicated Hospice Registered Nurse to join our team and provide compassionate, one-to-one patient care. You will assess needs, develop care plans, administer medications, manage pain, educate families, and collaborate with an interdisciplinary team; on-call rotation may be required.

Qualifications

• Graduate of an accredited nursing program with active RN licensure in Virginia.
• Minimum of 1 year nursing experience; hospice experience preferred.
• Knowledge of hospice philosophy of care.
• CPR certification and valid driver's license with insured vehicle.

Responsibilities

• Assess and monitor physical, emotional, and psychosocial needs of patients.
• Create and implement hospice care plans aligned with patient goals.
• Administer medications, treatments, and nursing interventions.
• Provide pain and symptom management and comfort measures.
• Educate and support patients’ families and caregivers.
• Collaborate with an interdisciplinary care team.
• Maintain accurate, timely documentation of care provided.
• Participate in on-call rotations as required by the branch.

As a Hospice Registered Nurse You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required , 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
