Job summary
PruittHealth Home Health in Rock Hill, SC is seeking an RN Home Health Case Manager to deliver skilled nursing visits in patients’ homes and develop start‑of‑care plans. You will collaborate with an interdisciplinary team, supervise LPNs and aides, and document care with AI-assisted tools.
Qualified candidates have an active RN license, CPR certification, a valid driver’s license, and 1+ year of RN experience (home health or acute care preferred).

As a Home Health RN Case Manager, you will:
- Provide skilled nursing visits in the patient’s home
- Complete assessments, start‑of‑care visits, and care plans

- Highly organized with strong time‑management skills; able to multitask, prioritize a busy caseload, and work confidently with technology and electronic documentation systems
