Job summary

PruittHealth is seeking a compassionate RN for Home Health in North Carolina. The Home Health RN Case Manager will provide skilled nursing visits in patients’ homes, conduct assessments, initiate care plans, and collaborate with an interdisciplinary team.

Strong clinical support and autonomy are offered with flexible days and a focus on work‑life balance. The role requires an active RN license, CPR, a valid driver’s license, and 1+ year of RN experience (home health or acute care preferred).

Qualifications

• Active, unrestricted RN license required.
• Valid driver’s license and reliable transportation.
• CPR certification required.
• Minimum 1 year RN experience; home health or acute care preferred.
• Experience with aging or adult populations is a plus.
• Strong time-management and multitasking abilities.
• Comfortable with technology and electronic documentation systems (e.g., iPad).

Responsibilities

• Provide skilled nursing visits in the patient’s home.
• Complete assessments, start-of-care visits, and care plans.
• Collaborate with interdisciplinary care team.
• Supervise LPNs and Home Health Aides.
• Document care using an iPad with AI assist (OASIS experience a plus).
• Educate patients and families to promote independence.
• Ensure continuity, quality, and safety of patient care.

Pay Based on Experience ($74,880 - $90,480)

Monday - Friday, Flexible Dayshift Hours

Coverage Area: Dobson, Flat Rock, Mount Airy, Pilot Mountain, Toast, White Plains, Danbury, Germanton, Pinnaclie, Walnut Cover and surrounding communities throughout Surry and Stokes Counties.
