Job summary

Amedisys in Mount Pleasant, SC is looking for a part-time Registered Nurse Case Manager to support patients at home. You will perform patient assessments and collaborate with the care team to develop an effective plan of care.

Responsibilities include referrals, teaching self-care, supervising LPNs and HHAs, timely documentation, and participating in on-call duties and ongoing education. Competitive pay and benefits accompany a flexible, patient-centered role.

Qualifications

• 1+ year of clinical RN experience required.
• Current RN license valid for the state you work in.
• CPR certification current and valid.
• Valid driver's license with reliable transportation and liability insurance.
• Note: if less than 6 months RN experience, may require RN Intern program.

• Perform patient assessments and collaborate to develop a plan of care.
• Refer to other disciplines as needed and document rationale.
• Promote health and independence through teaching and rehabilitative measures.

• Complete documentation timely and accurately.
• Regularly communicate patient progress to clinical manager and team.
• Plan and provide staff education.
• Handle on-call responsibilities and services.
• Engage in clinical development and continuing education.

• Makes referrals to other disciplines as indicated by the patient's identified needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicate patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families as assigned.

• Other duties as assigned.

Qualifications

• One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.*
• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

*Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.
