Job description

Job Title: RN Coordinator - Cancer Center

The RN Coordinator for the Cancer Center plays an expanded role in assessing patient needs, planning and providing nursing care, and coordinating follow-up care for patients receiving treatment in an outpatient Hematology/Oncology setting. The primary responsibility is care navigation throughout the continuum of care, including patient education, triage, and care coordination.

This is a full-time role, 40 hours weekly, with rotational start times between 7:00 a.m. and 8:30 a.m. Candidates must have at least two (2) years of RN work experience.

The role requires maintaining competency in Chemotherapy and Biotherapy administration, demonstrated by completing and maintaining The ONS Chemotherapy and Biotherapy Certification. Pursuing OCN certification after completing the required hours is encouraged to further demonstrate expertise in this specialized nursing field.

Key Responsibilities:

1. Coordinate treatment plans for Hematology and Oncology patients across the continuum of care.
2. Provide ongoing patient care and education within the adult Hem/Onc Department.
3. Serve as a resource for patients and families regarding diagnosis perception and medical care plans, as designated by the physician.
4. Act as a patient liaison to address issues, obstacles, or changes to their care plan.
5. Perform pre-visit RN triaging for new and returning patients to facilitate decision-making and initiate treatment planning.
6. Assist with supportive care referrals, including Palliative care, Psychology, and social work services.
7. Complete and document initial chemotherapy teaching sessions in EPIC, complying with regulatory requirements.
8. Participate in performance improvement activities to ensure high-quality care and assist providers with documentation and compliance.
9. Assist in orienting new personnel to ensure proper training to support patients and providers.
10. Maintain ongoing patient communication and provide clear instructions to ensure understanding and adherence to treatment protocols.
11. Perform triage of telephone health issues and provide appropriate advice, acting as a liaison to physician recommendations.
12. Communicate test results (lab, radiology, etc.) to patients via telephone and electronic medical record notifications.
