Job summary

Allay Hospice in Sheboygan, WI is seeking a dedicated Registered Nurse to join our hospice team as an RN Case Manager. You will provide patient-centered care at a critical time and coordinate with an interdisciplinary team in a supportive environment.

Responsibilities include assessing patients, creating care plans, administering care, managing pain and symptoms, educating families, documenting accurately, and participating in on-call rotations.

Qualifications

• Graduate of an accredited nursing school with current licensure in the state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
• Possess and maintain current CPR certification in states where required.

• Direct nursing care: administering medications, treatments, and interventions.

• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
