Job summary

Allay Hospice in Sheboygan, WI is seeking an experienced RN Case Manager to provide patient-centered hospice care. You will assess physical, emotional, and spiritual needs, develop care plans aligned with patient goals, and administer medications and interventions.

You’ll collaborate with an interdisciplinary team, educate families, maintain thorough documentation, and participate in on-call rotation. A nursing license in Wisconsin and CPR certification are required, with 1 year of hospice or

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• 1 year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Must be a licensed driver with an automobile that is insured in accordance with state requirements.
• Possess and maintain current CPR certification in states where required.

Responsibilities

• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.

Qualifications
