Job summary

Apreva Hospice in San Diego, California is seeking a dedicated Hospice Registered Nurse to provide patient-centered care during a critical time in patients’ lives. You will assess needs, develop care plans, and administer treatments within a compassionate team setting.

Responsibilities include pain management, family education, and on-call coverage; prior hospice or similar experience is preferred, along with CPR certification and a valid nursing license.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
• Possess and maintain current CPR certification in states where required.

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patients wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Salary Range

USD $50.00 - $55.00 / Hour
