Job Description

Are you an experienced RN Case Manager looking for a new opportunity with a prestigious healthcare company? Do you want the chance to advance your career by joining a rapidly growing company? If you answered “yes" to any of these questions – this is the position for you!

Daily Responsibilities

• Provide long‑term care management to a frail, economically disadvantaged, culturally diverse older adults, disabled adults and caregivers in order to help them remain safely at home and to prevent premature institutionalization.
• Assess new clients and determine appropriateness for the program, certify the client as nursing home eligible, conduct assessments and reassessments for identification of client’s medical, physical and psychological needs.
• Ensure client’s needs are met through written care plan approved by the program supervisor and discussed verbally with the client and/or his representative.
• Provide ongoing health education to non‑clinical staff members.
• Present cases at and participate in weekly team case conference per guidelines.
• Discuss during case conference complex health care issues for teaching purposes.
• Develop and implement care plans for assigned caseload.

Requirements

• Must be Registered Nurse with clear and active CA license.
• Must have 2+ years of clinical experience.
• Must have 3+ years of experience with health assessments, level of care criteria, care planning, and case documentation.
• Previous work with older adults and conducting home visits highly desirable.
• Previous work with mental health and/or substance abuse issues desirable.
• Ability to work as a part of a team, while at the same time performing job independently.
• Must have strong computer and typing skills.

Hours for this Position

• Monday – Friday 8‑5.
