Job summary

Phoenix Home Care & Hospice is seeking a motivated RN or LPN to join our In-Home team in Kansas City, MO and surrounding areas. This part-time role offers meaningful patient care in the comfort of clients' homes, with flexibility and supportive in-home team culture.

Responsibilities include completing admissions and reassessments, nurse visits, medication management, and caregiver support. MO license and valid driver’s license required; home care experience is a plus but not required.

Qualifications

• Active RN or LPN license in Missouri is required.
• Home care experience preferred but not required.
• Strong organizational and self-management skills.
• Strong and compassionate customer service skills.
• Valid driver’s license.

Responsibilities

• Complete admission assessments and reassessments for in-home clients.
• Provide nurse visits for adults and seniors: vitals, medication management, biopsies? (if applicable) and labs.
• May provide supervisory visits for caregiver services.
• Provide instruction/education on medications.
• Demonstrate good time management and attention to detail.

Responsibilities:

• Complete admission assessments and reassessments for In-Home caregiver population.


• Provide nurse visits for adult and senior population: Taking vitals, medication management, monthly diabetic nails, and labs.


• May provide supervisory visits for caregiver services


• Provide instruction/ education on medication


• Good time management, detail oriented and self-starter

Requirements:

• Active Registered Nurse or Licensed Practical Nurse license in the state of MO


• Home Care experience preferred but not required


• Strong organizational and self-management skills


• Strong and compassionate customer service skills


• Valid driver’s license
