Long-Term Care Registered Nurse (LTC - RN)

Facility Information

• Facility Type: Skilled Nursing Facility (SNF)
• Number of Beds/Residents: 83 beds with 74 residents; 20 beds are designated for a locked dementia unit
• Electronic Medical Record (EMR) System: Point Click Care
• Nurse-to-Resident Ratios: 5 RNs to 74 residents on day and afternoon shifts; 1 RN to 74 residents on midnight shift
• Required Certifications: Basic Life Support (BLS), Registered Nurse (RN) license
• Common Diagnoses/Patient Types: Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), Diabetes Mellitus (DM), Rehabilitation, Dementia

Job Responsibilities

• Administer medications
• Perform comprehensive head-to-toe assessments

Experience and Skills

• Open to first-time travel nurses

Clinical Details

• Intravenous (IV) Therapy: IVs accepted for fluids or antibiotics

• Attendance at Team Conferences for Acute Inpatient Rehab: Not required

Supervisory and Physician Support

• Director of Nursing (DON), Charge Nurse, and House Supervisor present for each shift
• First and third shift supervisors available
• Assistant Director of Nursing (ADON) takes call
• Physician availability after hours: 24-hour access to Team Health

• Physical Therapy (PT) / Occupational Therapy (OT)
• Dietician
• Activities Coordinator

Additional Facility Information

• Respiratory Therapist (RT) On-site: Not 24/7; no vented patients, but BiPAP and CPAP are used

Shifts and Scheduling

• 8-hour shifts:
  • Days: 7:00 AM – 3:00 PM
  • Midnights: 3:00 PM – 11:00 PM
  • Nights: 11:00 PM – 7:00 AM
• 12-hour shifts:
  • Days: 7:00 AM – 7:00 PM
  • Nights: 7:00 PM – 7:00 AM
• Block scheduling allowed for agency nurses

Work Requirements

• Weekend Coverage: Every other weekend
• Holiday Coverage: May be required to participate in holiday rotation
• Call or Standby: None
• Floating: Required throughout the facility
